Eligibility for ServicesPatients are accepted upon the recommendation of their attending physician. To be eligible for Home Health Services, a patient must be under the care of a physician and require skilled medical service. Most insurance and Medicare allow patients to leave home, if doing so is a taxing effort, for short, infrequent outings, such as church services, doctor visits and special occasions.
PaymentReimbursements for most or all of KCH Home Health services are available from Medicare, Medicaid, and private insurance.
KCH Home ConnectionsThe purpose of Home Connections is to provide an in-home service to ensure continuity of care. Home assessments, by a registered nurse, are available for patients that may not qualify for home health services.
We can assess blood pressure, heart rate, weight, the cardio-pulmonary system and review medications. After assessments, appropriate referrals will be made.
